How to do the audit:

Title: Audit of Self-Administered Injectable Adrenaline Prescription in Primary Care

An online system for submitting and viewing your anonymised practice data in comparison with the averages for all participating practices is under development, and will be available soon.

In the mean time, you can get started with your audit by using the print forms which you can download here.

When the online system is activated, you will be sent log on details which will allow you to enter and view your data online.

To register and do your baseline audit:
  1. Please download and complete this form to provide data for the baseline audit of patients on the practice register who have had self-administerred adrenaline (Anapen or Epipen) prescribed in the last 5 years.
  2. Send the completed form to Dr Mark Levy at
To audit individual patients:
  1. List and anonymise all patients prescribed injectable adrenaline for self administration in the last 5 years.

    These patients can be identified by searching on the drugs Anapen 0.15 and 0.30 mg or Epipen 0.3 or 0.15 mg, or the generic equivalent of these drugs.

    We have provided an Audit summary sheet (click here to download) for use when listing identified patients for the audit.

    We suggest simply renumbering each patient from 1 upwards and that you use these numbers online as the patient identifier, but also keep a record of the patients' computer numbers on your system, so you can identify patients later if necessary. Of course, you could use your own practice computer number on the audit sheet, as this will not be identifiable to any outsider.

  2. Then use the individual patient data sheets (click here to download) to enter individual patient data.
After receiving you online log on
  1. Enter your practice baseline data online first.

  2. When your baseline data has been accepted, you will receive an email giving you the go ahead to enter each patient's data online.

  3. After entering baseline and patient data, you will be able to see your audit data, compared with other practices in the UK and abroad.

  4. Info from summary and report button. This report, available once 5 patients have been entered will be helpful in evaluating whether the practice needs to take any action regarding care for patients prescribed self administered injectable adrenaline.

  5. The summary report, will also be useful as a record of audit performed for your annual personal development plan, and also will serve as evidence of audit for use in appraisal interviews.
